Medical Mileage Reimbursement Form
In Florida the Employer/Carrier is responsible for reimbursing the claimant for mileage to and from authorized medical appointments, including physical therapy, doctor’s visits, diagnostic testing and trips to and from the pharmacy to pick up medications. The reimbursement rate is currently $.445 per mile.

Any employer that is not in the construction industry, must have insurance coverage if they employ 4 or more employees.
